Are you passionate about patient care and creating a positive first experience for those navigating chronic health conditions? We’re looking for a Patient Enrollment Specialist (PES) to join our growing team and play a key role in supporting the onboarding of patients into our Chronic Care Management (CCM) program.
About the Role
As a PES, you will serve as the first point of contact for new patients. You’ll be responsible for educating patients on CCM services, obtaining consent, creating the first care plan, and ensuring a smooth transition to the long-term care team. This is an excellent opportunity for someone who thrives in a collaborative, patient-centered environment and enjoys a mix of communication and administrative responsibilities.
What You’ll Do
•Conduct initial outreach calls to introduce the CCM program and obtain patient consent.
•Develop and document the first care plan in alignment with CCM requirements.
•Complete the initial CCM report before transitioning patients to their long-term Care Team Coordinator.
•Ensure all documentation is accurate, timely, and compliant with regulatory standards.
•Work closely with internal teams to support seamless patient transitions.
•Address any questions or concerns patients may have during the onboarding process.
•Support various administrative tasks related to enrollment and care coordination.

Schedule

•Monday–Friday
•8 hour day

•Eligibility for Benefits: Begins on the 1st of the month following 60 days of employment.

Compensation:
$17.00 - $19.00/hour + Performance-Based Bonuses
